“Over the past 4 years, our PACS system has grown tremendously and our vendor DR Systems has been a vital part of that growth. We have 10 sites, of those 3 are small hospitals, 3 are immediate care centers, 3 remote reading sites, and 1 medical center with level one trauma care. One of the remote reading sites is located 600 miles from the medical center in Greenwich, CT. The ability to read remotely has been a godsend to our radiologists. They have struggled to recruit new radiologists, but now are able to attract more potential radiologists for their group. They have become extremely productive and have more flexibility in covering all the sites, without having to be onsite to get their work done. We have interfaced our RIS system, Cerner Quadris, with the DR Systems PACS. The order in Quadris is passed to the PACS via the RISGate interface. Then the same order number is sent back to Quadris, with a mouse click, for digital dictation at the time the radiologist begins his dictation. On the surface it appears like it is all one system. Another outstanding feature that the referring physicians really like is the ability to pull up the report on the image that they are currently viewing. It really streamlines the process of getting the information they need to treat their patients. Of course, I can't say enough about access to image data via the DR Systems Web Ambassador software. Specialty physicians, such as orthopedic surgeons and neurosurgeons, love to be able to see images from their home. They can now start treatment on patients sooner, which many times means better overall outcome for the patient. We have become filmless and now paperless with the magic of document scanning. Now we can scan in any type of document that is pertinent to the radiologist reading the image. This has increased the overall turnaround time because we are not shuffling around paper. We have increased our workload, but have been able to decrease our clerical staff by 3 FTE's. Fast, quick, and easy access to image data has become the standard of care throughout Borgess Health Alliance.”

“For the past two years we have been deeply involved in the PACS selection process. We formed a PACS Selection Committee and submitted RFP’s to four PACS Vendors. The PACS Selection Committee met on a weekly or bi-weekly basis for over a year, and narrowed the selection down to two vendors. Each member of the PACS Selection Committee had specific responsibilities which they followed through with each vendor, compiled the vendor’s responses and shared these with the other members of the PACS Selection Committee. Site visits were initially performed at numerous sites and then additional site visits were deemed necessary with the Final Two Prospective Vendors: GE Medical Systems and DR Systems, Inc. As a member of the PACS Selection Committee meeting, I am pleased to report that we have selected DR Systems as our Vendor of Choice. Although the selection process was long, arduous, and at times difficult to determine “exact” differences and advantages between the two finalists, the final voting results of the PACS Selection Committee was unanimous for DR Systems, Inc. DR Systems will take us filmless, with the exception of mammography, approximately 1-2 weeks following training and installation. Our purchase order includes five multiple 5-monitor flat panel Dominators, 11 CR Systems, the new Reporter for Digital Dictation with Voice Recognition, and enough Ambassador Software (LAN and WEB) for over 300 display multi-tasking Clinical Review Stations. Locations in the ER Trauma Rooms and Surgery will be wireless. This also includes a 5+TB DVD Archive. We look forward to a long and mutually satisfying relationship as partners and friends with DR Systems, Inc. “Over the last quarter of a century, advances in medical imaging have touched and often redefined virtually all fields of medicine. The addition of computers in medicine has completely changed the role of the radiologist. …Until recently, digital imaging studies have been viewed and archived on radiographic film. Image transfer was limited to digitization of filmed images and their transfer by proprietary systems. The amount of data lost due to conversion to and from analog film and the inherent limitations of film display and storage has led to the development of PACS (Picture Archiving and Communications Systems). These systems allow the imaging studies to be viewed, manipulated and archived in their digital form. In the early 1990s the American College of Radiology worked hard to establish a standard for imaging systems. DICOM (Digital Imaging Communications in Medicine) was accepted as the standard format for image acquisition, display and archiving. With this standard, digital imaging was no longer vendor specific. Images could be viewed, manipulated, stored and transmitted in the same language. With high-resolution display devices, these images could be viewed in their digital form at a computer workstation. Early in this transition, both Jewish Hospital administration and the radiologists recognized the inherent advantages of PACS. Former Director of Medical Imaging, Dr. J. Wendell Tyson, and Senior Vice President and Chief Information Officer, David Pecoraro, chose a company from San Diego to provide a PACS system. DR Systems provided a user-friendly display system suitable for image interpretation and archiving. They also provided a simple PC-based platform adaptable to Jewish Hospital’s existing network infrastructure. Images could be viewed throughout the Jewish Hospital intranet and were accessible not only in the imaging department but throughout the hospital and the physicians offices. … I am pleased to say that we are now, with the exception of mammography, a completely digital and filmless department. Recognizing that images need to be reviewed outside of the hospital environment, we have explored alternatives to filming of studies for review outside of the Jewish Hospital network. Due to the size of the digital images, conventional phone transfer of images was impractical. The World Wide Web was chosen as the vehicle for data transfer. While accessible, the Internet presented security problems both for the hospital network and the patient. Proposed HIPPA regulations enacted by the Clinton administration mandate confidentiality and privacy of patient records. Furthermore, the Web uses http as its language, not DICOM. With these concerns in mind, Jewish hospital and DR Systems installed a Web server in late 1999. This server has been used and extensively tested by the radiologists at night over the last year. Images are encrypted and access protected by password. The use of the site requires signed confidentiality agreements. …In addition to the images, a preliminary voice interpretation is included. Images can be viewed using either Netscape or Internet Explorer. The images are compressed for speed of data transfer but high-speed Internet connections are preferred. In Louisville, both DSL and cable modems are available. Digital imaging, PACS and Internet-based image review is the wave of the future in medical imaging. Jewish Hospital’s continuing commitment to new technologies has placed our department well ahead of the curve.”

“As the radiologist at our 450-bed hospital with the prime interest in developing a PACS system - with the support of our administration - we began to look at possible vendors and solutions, especially to our lost film problem about two years ago. Our introduction to DR Systems was by happenstance at a PACS meeting where there was a small DR system set up for demonstration. As we watched a radiologist become familiar with the system and handling data in only a few minutes, it was immediately obvious that this was a very user-friendly system. We then began to investigate more fully and after several site visits to other vendor’s installations as well as to a DR installation in San Diego, we were convinced that DR Systems was not only the most cost-effective system available but probably one of the best systems for its total versatility. Within the next year, we made an initial presentation to our Board of Directors for a Phase I purchase to be followed with a Phase II during the next fiscal year. The Board was so easily convinced that this was the direction to proceed that both Phases were approved at that time. We were then faced, as was DR Systems, with doing more than we had initially thought feasible in that time frame. By mutual agreement, the whole installation was spread out over about 6 months, the end result being a totally digital hospital except for mammography. We are now connected for conferences, in surgery, and throughout the hospital with images being readily accessible almost as soon as the study is completed. The radiologist turn-around time for reading the study in many cases has become much faster than a previous “stat” read. Many studies have written reports faxed to a clinicians’ office before the patient can even return from having the study performed. There have been virtually no lost images and complaints about the file room are now nonexistent. With the release of the Web Ambassador, we have connected all the radiologist’s homes via cable modems over the internet with studies being available within minutes of completion. Similarly, most of the clinicians’ offices are now being connected via a local area network (LAN) utilizing the Web Ambassador, and they will have access to the studies also upon completion. The voice clip and montage will also be available after the radiologist has interpreted the study. DR Systems has been very responsive to our needs and helping to resolve issues specific to our installation. We can deal directly with the programmers to resolve issues. Many suggestions I personally have made have subsequently been incorporated in the next software release. I feel that DR Systems is very interested in the input of their end users and I consider the leadership of that company as friends, not just as a vendor customer relationship. I have never had a request or concern that was not answered quickly, often the same day. I take pride in the success we have had with DR Systems and happily recommend them for consideration in any PACS application.”
